</b><span style="font-weight: 400;">Colchicine is one of the oldest anti-inflammatory drugs in continuous clinical use, a molecule derived from the autumn crocus with no patent protection anywhere in the world and no meaningful branded exclusivity in Egypt. The class is small in absolute value — the Egyptian colchicine market is estimated at approximately EGP 118 million on a Moving Annual Total (MAT) basis to December 2025, up from around EGP 65 million in 2021, a four-year value CAGR of roughly 16 percent. Unlike most Egyptian therapeutic classes, its growth is largely real: unit demand has expanded steadily as gout prevalence rises in step with metabolic syndrome, and as three distinct clinical crossroads converge on the same generic molecule. The first crossroad is gout: still the dominant indication, still growing quietly with population and diet. The second is Familial Mediterranean Fever (FMF), a recessively inherited autoinflammatory disease with elevated prevalence in populations of Eastern Mediterranean origin — including a documented Egyptian patient pool — for which colchicine remains the only proven prophylactic therapy. The third is cardiovascular repositioning: the LoDoCo2 (NEJM 2020) and COLCOT (NEJM 2019) trials established low-dose colchicine as an evidence-based anti-inflammatory adjunct in chronic coronary disease and post-myocardial-infarction settings, opening a small but expanding new indication share. The class expanded from roughly EGP 144 million in 2021 to approximately EGP 3.26 billion on a Moving Annual Total (MAT) basis to December 2025 — a four-year compound annual growth rate (CAGR) of about 118 percent. Critically, and in sharp contrast to the mature core of the market, this growth is majority real volume rather than a currency artifact: unit demand is expanding rapidly, driven by Egypt’s exceptionally high burden of type 2 diabetes and obesity. The class is structured as an originator duopoly. MOUNJARO (tirzepatide) and OZEMPIC (semaglutide) together define the category, with MOUNJARO effectively doubling from a standing start and OZEMPIC growing units about 61 percent. Both brands are supply-constrained rather than demand-constrained — the binding limitation is manufacturing allocation and price accessibility, not patient appetite.
